Closed School Records FAQs
Who can request student records?
Only the person whose name is on the record can request their student record. If the person requesting records is not the same as the person on the school record, please send a waiver from the person along with the appropriate fee.
How do I request records?
Anyone requesting their student records from the Diocesan Archives should download the Student Record Authorization Form and return it to the Diocese with the appropriate fee.
If the record request is for a student under age 16, the request must be made by that child’s parent or guardian. Click here for the Parents Request of student records form.
Please note that only money orders will be accepted. Personal checks are not accepted.
All requests must be sent via mail. If for some reason there's a need to drop off or pick up forms from our office for record requests, please call in advance.
Background Checks
Research or Investigation Companies seeking to verify past student enrollment for background checks should be aware that Diocesan Archives does not verify educational information by either phone or fax.
A copy of a waiver from the person whose record is being requested, along with a bank check payable to “R.C. Diocese of Brooklyn” in the amount of $10.00, should be sent to:

Genealogical Requests
School records can be an important source of genealogical information. Any student record requests for this purpose must be accompanied by documentation of the person’s death in the form of a death certificate. Please also send a donation of at least $25.00 to defray the cost of researching your request. Make your check or money order payable to “R.C. Diocese of Brooklyn” and mail it to the following address:
